You may be eligible if your GP has documented a chronic condition, such as diabetes or heart disease, that has been or is likely to be present for six months or longer.

Your GP will assess your condition and, if appropriate, can write a referral under the GP Chronic Condition Management Plan (GPCCMP). This referral allows you to access up to five subsidised sessions per year with allied health professionals.

If you already have an existing GP Management or EPC Plan from before July 2025, it will remain valid until 30 June 2027 before transitioning fully to the GPCCMP.

The rebate is available for services provided by eligible allied health professionals, such as Physiotherapists, Chiropractors, and Dietitians, as long as these services are included in your GP’s care plan.
